PLEASE TAKE NOTE OF THE FOLLOWING TERMS AND CONDITIONS: 

 

·         all booths must be equipped with a 5lb fire extinguisher.  failure to produce a fire extinguisher on inspection will result in revocation of license no exception

·         tent size is limited to a 10 x 10 tent only – PLEASE ENSURE BOOTH IS PRESENTABLE

·         no drinks should be served in glass bottles – PLASTIC CUPS ONLY – DISPLAY GLASS BOTTLES SHOULD BE AT BACK OF TENT not accessible to the general public

·         CLEAN TENT - ALL GARBAGE MUST BE SECURED IN PROPER GARBAGE BAGS

·         food handler’s certificate must be attached to this form and visible at event booth

·         must have fresh potable water for hand washing

I HEREBY ATTEST AND DECLARE by affixing my signature or mark:

 

1.                  that the aforementioned is true and correct (either read to me or by me)

2.                  That my food handlers certificate and liquor license are up to date

3.                  That I have a (Five) 5lb cylinder fire extinguisher

2.            I have read and agreed to the terms and conditions set out on this application form.

 

I also understand that the antigua & barbuda festivals commission (abfc) reserves the right to refuse my application and that any false statements or misrepresentation I have made ARE sufficient GROUNDS for my disqualification from THE CARNIVAL VENUE.
