ANTIGUA & BARBUDA FESTIVALS COMMISSION
APPLICATION FOR VENDING

PLEASE TAKE NOTE OF THE FOLLOWING TERMS AND CONDITIONS:

  •  ALL BOOTHS MUST BE EQUIPPED WITH A 5LB FIRE EXTINGUISHER. FAILURE TO PRODUCE A FIRE EXTINGUISHER ON INSPECTION WILL RESULT IN REVOCATION OF LICENSE NO EXCEPTION.
  •  TENT SIZE IS LIMITED TO A 10 X 10 TENT ONLY – PLEASE ENSURE BOOTH IS PRESENTABLE.
  •  NO DRINKS SHOULD BE SERVED IN GLASS BOTTLES – PLASTIC CUPS ONLY – DISPLAY GLASS BOTTLES SHOULD BE AT BACK OF TENT NOT ACCESSIBLE TO THE GENERAL PUBLIC.
  •  CLEAN TENT - ALL GARBAGE MUST BE SECURED IN PROPER GARBAGE BAGS.
  •  FOOD HANDLER’S CERTIFICATE MUST BE ATTACHED TO THIS FORM AND VISIBLE AT EVENT BOOTH.
  •  MUST HAVE FRESH POTABLE WATER FOR HAND WASHING.

I HEREBY ATTEST AND DECLARE BY AFFIXING MY SIGNATURE OR MARK:

  1. 1. THAT THE AFOREMENTIONED IS TRUE AND CORRECT (EITHER READ TO ME OR BY ME)
  2. 2. THAT MY FOOD HANDLERS CERTIFICATE AND LIQUOR LICENSE ARE UP TO DATE
  3. 3. THAT I HAVE A (FIVE) 5LB CYLINDER FIRE EXTINGUISHER
  4. 2. I HAVE READ AND AGREED TO THE TERMS AND CONDITIONS SET OUT ON THIS APPLICATION FORM.

I ALSO UNDERSTAND THAT THE ANTIGUA & BARBUDA FESTIVALS COMMISSION (ABFC) RESERVE THE RIGHT TO REFUSE MY APPLICATION AND THAT ANY FALSE STATEMENTS OR MISREPRESENTATION I HAVE MADE ARE SUFFICIENT GROUNDS FOR MY DISQUALIFICATION FROM THE CARNIVAL STREET LAUNCH VENUE
